This Symposium is focused on the radiation oncologists’ perspective on the treatment of prostate cancer recurrence after previous radiotherapy. Local salvage options have historically been considered to be extremely toxic, with limited options beyond radical prostatectomy. However, a synthesis of the literature and promising new results from prospective studies suggest that specialized forms of re-irradiation may be safe and effective. A careful diagnostic workup is necessary in order to decide upon the therapeutic modality. Multiparametric MRI must be interpreted with caution in order to differentiate the recurrent disease from radiation effects on normal tissues, but machine learning algorithms may improve the target definition in focal salvage. PSMA PET advantages and limitations, will be discussed in this context. The need of a biopsy to confirm local recurrence is still a debatable topic. Re-irradiation by SBRT and Brachytherapy protocols, outcomes and toxicity will be presented from the perspective of current data and speaker’s personal experience, highlighting discussion of future perspectives.

Hypoxia is the most cited biological topic in translational radiation oncology. The microenvironment of solid tumors is hypoxic compared with normal tissue, and this hypoxia is associated with decreased radiosensitivity. Irradiation of the tumor microenvironment causes differential activation of pro-survival and pro-death pathways in malignant, stromal, endothelial and immune cells, increasing tumor-infiltrating immune cells and causing a profound cellular and biological reconfiguration via multiple, non-redundant mechanisms. At present, various approaches have been investigated to support a high level of evidence for the benefit of hypoxic modification. We will highlight the biological rationale, and the current investigations, related to identifying and overcoming hypoxia in modern radiotherapy.

In this session featuring key opinion leaders Drs. Fiona McDonald, Dirk De Ruysscher, Corinne Faivre-Finn and Ben Slotman we will focus on state-of-the-art radiation therapy in lung cancer looking at techniques being utilised in oligometastatic disease and when to apply them along with updates in the exciting field of combination regimens of immunotherapy and radiation. We will review where things stand with the role of prophylactic cranial irradiation in lung cancer and have a robust discussion of the intersection of radiomics and radiotherapy.
