Management of radio-recurrent prostate cancer

This Symposium is focused on the radiation oncologists’ perspective on the treatment of prostate cancer recurrence after previous radiotherapy. Local salvage options have historically been considered to be extremely toxic, with limited options beyond radical prostatectomy. However, a synthesis of the literature and promising new results from prospective studies suggest that specialized forms of re-irradiation may be safe and effective. A careful diagnostic workup is necessary in order to decide upon the therapeutic modality. Multiparametric MRI must be interpreted with caution in order to differentiate the recurrent disease from radiation effects on normal tissues, but machine learning algorithms may improve the target definition in focal salvage. PSMA PET advantages and limitations, will be discussed in this context. The need of a biopsy to confirm local recurrence is still a debatable topic. Re-irradiation by SBRT and Brachytherapy protocols, outcomes and toxicity will be presented from the perspective of current data and speaker’s personal experience, highlighting discussion of future perspectives.
